Before beginning be sure to have a photo of every diver’s certification card available to you for upload onto this submission form. If you are not certified obviously no card photo will be needed.

Rental Equipment Replacement Cost
One or more of the divers in your group has indicated they will require rental equipment from Salty Endeavors. In the event the equipment is lost, damanged, or stolen, your credit card on file will be used to compensate us for replacement costs. Those costs are the following: Mask $50 Snorkel $20 Regulator First Stage $120 Service fee for flooded, or $200 for replacement Console consisting of an Analog depth guage and pressure guage $250 Regulator second stage x 2 $150 each Buoyancy Compensator $500 Fins $200 Boots $50 Wetsuit $150 Wrist Computer $250 Torch $150 GoPro $600
I understand the replacement costs for lost or damaged equipment, and understand I will be billed accordingly in the event of lost, damanged, or stolen equipment*
